What We Do
Cogni Care develops and delivers personalized rehabilitation and support services to people experiencing cognitive change. Our approach is goal driven and outcome focused; we tailor services to maximize rehabilitation potential, promote independence, support aging in place and facilitate community integration.
Cognition: the mental processes involved in gaining knowledge and comprehension, including thinking, knowing, remembering, judging and problem-solving.
What is Cognitive Change?
Cognitive ability differs between individuals and changes naturally throughout life. Some changes enhance skills, such as learning something new or becoming more effective at something through practice. Other changes can diminish skills, such as becoming “rusty” with something we haven’t done in a while or discovering that our memory or speed of processing may decline as we age.
Changes with cognition may also occur as a result of acquired brain injury, dementia, addictions and mental and physical illness or may be present at birth due to a congenital disorder. These types of changes can challenge our ability to live independently, secure and maintain employment, fulfill family obligations or effectively integrate into our community.
Where Do We Provide Services?
Cogni Care provides services throughout Vancouver Island, British Columbia. With special arrangement, Cogni Care can establish and deliver services in most areas of the province.
We work with people in the community and not from a clinic. Our services are delivered within people’s homes, places of work, local recreation centers and surrounding community.
Our administrative offices are located on the unceded traditional territories of the W̱SÁNEĆ peoples. With respect we acknowledge the lək̓ʷəŋən, Songhees, SXIMEȽEȽ (Esquimalt), W̱SÁNEĆ, BOḰEĆEN (Pauquachin), SȾÁ,UTW̱ (Tsawout), W̱JOȽEȽP (Tsartlip), W̱SÍḴEM (Tseycum), MÁLEXEȽ (Malahat), T’Sou-ke (Sooke), and Scia’new (Beecher Bay) peoples, on whose homelands we carry out our work. Cogni Care recognizes the stewardship of Indigenous peoples of these lands and with humility, we continue to work toward building our relationship.
